Distance measurements on hills

Good evening from Italy and congrats for this helpful site, it's a treasure for runners!
I have a question for you, are distances measured like lines are flat or considering also the gradient of roads?
As an example, considering a climb as a right triangle, length is calculated on the hipotenuse or on the long cathetus? Because this would not be a problem in flat routes, but makes difference in hilly routes.

Distances are measured as if the route is flat.
